• Exposure and infection reporting

  Report a positive COVID-19 test result:

  • Students who have tested positive for COVID-19 should report via the Hamline student COVID-19 self-report Google Form.
  • 工作人员 and 教师 who have tested positive for COVID-19 should report via the Employee COVID-19 Self-Report Form.
  • 使用 CDC isolation calculator to determine your appropriate isolation time.
  • Notify any close contacts so that they may monitor themselves for symptoms, 戴口罩, 或者去做检查.
  • Determine your close contacts with the CDC close contact guide. You may also notify your close contacts anonymously on tellyourcontacts.org.

  What to do if you were exposed to COVID-19?

  • Wear a high-quality mask around others for 10 days after the exposure
  • On day six, take a COVID-19 test
  • If you develop symptoms at any time, take a COVID-19 test

  Symptoms of COVID-19 may include:

  • Shortness of breath or difficulty breathing

  • Muscle or body aches

  • New loss of taste or smell

  Where can I get COVID-19 Testing?

  • COVID tests can be found at local pharmacies or you can call you health insurance company for more information.
• Vaccination reporting requirements and forms

  COVID vaccination is strongly recommended but not required for students, 教师 and staff.  

  Vaccinations are available nearby at Lloyd’s Pharmacy (720 Snelling Ave N, St 保罗、锰 55104; phone: 651-645-8636) or your primary care provider.
